I've been all over reading a million different people and their thoughts, reactions, etc to the episode so I have no idea where this came from. No doubt I got the idea from someone else, because I hardly ever have thoughts of my own.
I'm now going to freak out over things that are purely speculative. Because it's what I do.
I am unspoiled past 4.14 and want to stay that way, so this is speculation, but it makes so much sense.
Whatever Ruby wanted Sammy to do, be it drink demon blood or have more sex with her or whatever in order to increase his powers, he's already doing it, and he's already turning evil because of it.
Which would explain why he threw his phone across the room, which didn't seem to make sense once we knew he wasn't under the siren's influence at that point. Also, it explains his cruel mocking of Dean's suffering in hell. I actually liked the grownupness of his casual sexual encounter with the doctor, but Dean thought it was OOC and so perhaps that too was a symptom of his eviling? I think that's kind of annoying and sexist, which, uh, yeah, that's probably what the writer meant. Gentle little Sammy would never have casual sex, because casual sex is so wrong. Perhaps Sam's saying his only wish was to have Lilith's bloody head on a plate from Wishful Thinking was already evidence of his falling into the dark side? It makes a lot of sense.
I kind of like Sam going evil as long as Dean gets to save him, but what I don't like here is that it seems to me that must mean that Ruby is evil. I know most everyone hates Ruby, but I don't, and I so did not want her to be evil. I loved the idea of a female character with a dark past struggling to overcome her own evil and banish it, kind of like a female Angel from btvs. We never get a complex female character like that, and I really wanted Ruby to be that for me.
I always wondered why Lilith would trust Ruby and let her return to earth. If she has been working for Lilith all along, it makes sense.
I will be okay with this as long as whatever Sam is doing to become evil is not having sex with Ruby. But it probably is the sex, and that is just fucking lame.
So if Sam is going evil (and I think he is) does Dean realize it? Does he recognize that the changes in little brother are due to supernatural influence rather than natural ones and needs his help? Because I still remember how quickly Sam knew that Dean was not himself in Skin. Sam needs the Dean from buabs, the one totally focused on protecting and rescuing him, the one who wouldn't be swayed no matter what awful things Sam did.
Please let Dean get it, and please let Sam be drinking demon blood or something cool like that, cause I am going to be really upset if it's all about sex.
